This is a young patient, still in her 40s, who developed worsening vision when her posterior polar cataracts became worse, leading to the spoke-like opacities that are seen radiating from the center of the posterior plaque. In these eyes, we must be careful because the posterior capsule is weak and fragile and surgeons cannot use the standard technique of cataract surgery. Here we use the visco-dissection technique to carefully remove the cataract while protecting the posterior capsule.
