From Hyperarousal to Receptivity: Enhancing Ketamine Outcomes Dr Drozd observed at Lucid Therapeutics that many ketamine and Spravato patients improved on depression scales — yet continued to struggle with insomnia, anxiety, and hypervigilance. Mood was lifting, but the nervous system remained in survival mode. This led to a critical insight: treatment response may depend on physiological readiness. Ketamine promotes neuroplasticity and cognitive flexibility, but persistent sympathetic activation can limit emotional openness and therapeutic depth. We began integrating right-sided Stellate Ganglion Block (SGB) for patients with residual hyperarousal. In a 36-patient case series, adding SGB significantly reduced PTSD and anxiety symptoms — and unexpectedly enhanced the depth and emotional meaningfulness of subsequent ketamine sessions. We didn’t change the ketamine. We changed the nervous system entering the session. This supports a sequenced care model: Regulation → Receptivity → Rewiring By calming threat circuitry first, patients may become more emotionally accessible — allowing psychedelic-assisted therapy to work more effectively and durably.
