Optic atrophy is a term that describes a structural change in the optic nerve. It is usually associated with loss of healthy nerve tissue. Optic atrophy can leave the nerve looking pale (pallor) or cupped (cupping) from lost and/or damaged nerve fibers. Clinical symptoms and signs of atrophy may include- loss of visual acuity peripheral visual field loss color desaturation or loss loss of contrast (brightness) In some patients with optic atrophy, there may be no associated symptoms and the finding is simply picked up on a routine dilated exam. Optic atrophy may arise from a number of optic nerve conditions such as- glaucoma (open angle, closed angle, or normal tension) lack of oxygen to the optic nerve (aka ischemia, or a type of optic nerve stroke) prior inflammation of the optic nerve (I.e., optic neuritis) compression of the nerve from a tumor or mass lesion trauma to the optic nerve (traumatic optic neuropathy) prior swelling of the nerve from papilledema (post-papilledema optic atrophy) There are also retinal conditions that lead to secondary atrophy of the optic nerve, such as a retinal stroke (CRAO, BRAO, CRVO, or BRVO) or retinal inflammation.
