Join us in this lecture where Professor Zach Murphy will discuss the vestibulocochlear nerve, cranial nerve VIII, focusing on the auditory pathway. We begin by outlining the dual function of this nerve, emphasizing its cochlear division for hearing and vestibular division for balance. The lecture details the origin of the nerve at the pontomedullary junction, its entry into the internal acoustic meatus alongside the facial nerve, and its separation into the cochlear and vestibular branches. From there, we explore the auditory pathway starting at the organ of Corti within the cochlea, where sound vibrations are transduced into electrical signals. These signals travel via the spiral ganglion neurons to the cochlear nuclei in the brainstem, then ascend bilaterally through the superior olivary complex and lateral lemniscus to reach the inferior colliculus. The pathway continues to the medial geniculate nucleus of the thalamus before projecting to the primary auditory cortex in the temporal lobe, where sound perception and interpretation occur. We also discuss how this bilateral projection helps preserve hearing in one ear even after central lesions and outline the role of the vestibular division in transmitting information from the semicircular canals, utricle, and saccule to maintain equilibrium. Clinical considerations include how lesions at different levels of the pathway can cause unilateral sensorineural hearing loss, tinnitus, or balance disturbances, and how specialized testing like audiometry and caloric testing can localize dysfunction.
