Arthroscopic repair can achieve osseous union and normalization of glenoid anatomy with a low prevalence of recurrent instability1 even in shoulders with a chronic, large glenoid defect as long as the shoulder retains an osseous fragment2-4. We attribute these favorable outcomes to excellent fragment reduction and retensioning of the entire inferior glenohumeral ligament (IGHL) enabled by an extensive labral release during the procedure1,5-7.
