Hello dear patients. Today we are going to review the case of white spots on foreskin. A young man came to see us, he had white spots on foreskin, and moreover, they had densification at their base. This bothered the patient, you can see them on the picture. They are a bit whiter than the skin surrounding the foreskin itself, considering that it is quite thin. They had clear densifications at their base, so they basically were scar tissue. At first we thought that this is sclerotrophic lichen, or a disease of autoimmune nature that leaves scars after it. This is the case when an immune attack on the skin takes place and scars are formed. Today it's debated whether this disease is linked to herpes genitalis in some cases etc. However, such cases, when there is such a link, are rare. Later on, during questioning of the patient, it turned out that this patient had allergy for a long time. After observation, without analyses, dermatologists prescribed glucocorticosteroid creams. The examples of such creams are tetraderm, advantan, triderm etc., as well as dermavit – the strongest hormonal cream. Such creams have side effects and as the patient took them for a long time, then these side effects combined into one side effect – skin atrophy, atrophy of foreskin. Most commonly, the patients pay attention when there is an atrophy of facial skin as there it is more visible. Skin turns into a brick-red color, it looks like a brick. So, a superficial atrophy appears. Here, the atrophy appeared after long-term use of hormonal glucocorticosteroid creams, and considering that there was a densification at the base of these spots, then it obstructed a sexual contact. What to do in such cases? First of all, the use of glucocorticosteroid cream has to be discontinued. Examination and treatment have to be done but for now we are not going to talk about it as this video is about what to do in particular with these white spots. Circumcision can be done, however, at that age, after 20, it's quite difficult to convince an adult man to do such surgery without any apparent reasons. So, here, a local use of liquid nitrogen can be used, it has to be done with caution. We use krioiney device with its small tips to have a local influence on these spots, and we did it in this case. Another option is photodynamic therapy. Here, however, we chose the simplest method and the most important thing is that the densification in the focuses of this atrophy disappeared. Because of that, the person no longer felt unpleasant sensations during sexual contact. So the aim was achieved and this patient did not do circumcision. However, it would have been better not to get to this state and the problem of penis reddening can be successfully solved not only by hormonal medications, but by consultations of allergologist and by general treatment; not only be external therapy containing hormones that, as we can see, induces complications.
