Double Chop: Explanation of Phaco Tip Position for Dense Lenses

Double Chop is a modified chop technique which defragments the lens without the need for any U/S or vacuum. The technique utilizes mechanical vector forces to fracture the lens by placing the chopper out to the equator in typical horizontal chopping fashion and then placing the phaco tip subincisionally into the epinuclear material at the level of the equator. Both instruments are brought centrally and fractures the endonucleus with horizontal vector forces. So how can you double chop if the lens is so dense that the phaco tip cannot be buried into the subincisional epinucleus? You place the tip on the surface of the lens subincisionally, then with diagonal vector forces, you can still safely, effectively, and consistently fracture the lens. There is no zonular stress and no tilting of the lens due to centripetal vector forces which makes this a safe and gentle maneuver without needing U/S or vacuum.
