😂 I got so many comments on this story that I had to turn it into a full post. Full disclosure: not a patient I’m caring for, sent this picture from a friend, but great illustration of the basic Arthroplasty concepts of #leglength and #offset 🏀 This is the #ortho equivalent of an “air ball”. The canal was filled with cement in preparation for a cemented stem and then the stem was inadvertently placed medial to the femur rather than into the cemented canal. How this happened, I have no idea. 📐 The review concept here for medical students and pre-meds and maybe some junior orthopedic residents that haven’t done Arthroplasty yet, is regarding leg length and offset. This “construct” if you can call it that, has increased offset, meaning the distance laterally from the pelvis to the femur is increased. To increase leg length, you would move the femur vertically more distal away from the body. 🍗 In total hip replacement these two principles are used to optimize abductor muscle tension and efficiency and allow for a stable and well functioning hip. 🙋♂️ Can you guess how this happened?
