Yüksek riskli prostat kanseri, bu hastalığın muhtelif durumlarını tanımlayan bir antitedir. Yüksek riskli kanser genellikle, hastalığın klinik relaps, tedaviye bağlı komplikasyon gelişmesi, sekonder malignansi gelişimi, kanser spesifik mortalite gibi durumların yüksek olduğunu belirtir. Prostat kanserinde yüksek risk tanımını iyi yapmak ve hangi hasta grubunun bu tanımlamaya dâhil olacağını çok iyi belirlemek gerekmektedir. Özellikle son yıllarda yapılan çalışmalar sonucunda risk seviyelendirme teknikleri gelişmiş, bu sayede prostat kanserinin doğal seyrinin tahmini daha kolay yapılır hale gelmiştir. Prostat kanserinin tedavisinde, radikal prostatektomi, radyoterapi, hormon tedavisi ve kemoterapi gibi farklı yöntemler uygulanmaktadır. Ancak yüksek riskli prostat kanserinde hangi yöntemin hangi hasta için daha uygun olduğunu söylemek zordur. Yapılan çalışmalarda yüksek riskli kanser tedavisinde tek bir yöntemin değil de birden çok yöntemin kombine edildiği multimodal tedavi yöntemlerinin daha faydalı olduğu bildirilmiştir. Bu nedenle klinisyen tarafından prostat kanseri tedavisinde kullanılan yöntemlerin çok iyi bilinmesi ve doğru hasta grubuna doğru tedavi kombinasyonunun doğru zamanda uygulanması gerekmektedir. High risk prostate cancer is an entity representing diverse types of this disease. Generally, high risk cancer defines to be increased situations such as clinic relaps, complications due to treatment, occurence of secondary malignancy or cancer spesific mortality. In prostate cancer it is crucial to correctly define the high risk disease and determine the patient population involved in this group.With the studies established especially at the last few years, risk stratification techniques were developed so predicting the natural history of prostate cancer became easier. Radical prostatectomy, radiotherapy, hormone therapy and chemotherapy are different procedures being used for the treatment of prostate cancer. But in prostate cancer it is difficult to choose which procedure is most suitable for the individual. According to the recent studies, multimodal treatment options combining more than one procedure are more beneficial in the prostate cancer treatment, instead of single procedure.
