100% of cataract surgeons will experience the complication of a ruptured posterior capsule and then having the nucleus drop into the vitreous cavity. When the nucleus drops into the vitreous cavity during cataract surgery, it is essential to remain calm and avoid further manipulation that could worsen vitreous traction. Immediately inject a dispersive viscoelastic to pressurize the anterior chamber tamponade the vitreous before withdrawing the phaco probe. Perform a thorough anterior vitrectomy to remove prolapsed vitreous, using triamcinolone if needed to visualize strands. Do not attempt to retrieve nuclear fragments with the phaco probe, as this risks retinal damage. Once the anterior segment is stable and vitreous cleared from the wound, close the eye securely. The patient should then be referred promptly to a vitreoretinal surgeon for pars plana vitrectomy and posterior segment removal of the dropped nucleus.
