The improvement of interventional materials and technologies over the years allows today tissue sampling from potentially every area of the body. The introduction of forceps in the field of endocavitary biopsies is a well know and established tool that finds major application in biliary and urinary districts. Only few evidences describe the use of forceps for endovascular biopsy procedures. The endovascular approach has potential advantages over percutaneous biopsies because the crossing of vital structures is avoided and all districts are approachable if adequate vascolarization is present. In addition, endovascular procedures usually show lower pain compared to percutaneous one. This video reports a complex case of biopsy of a pelvic mass infiltrating the left iliac vein with subsequent complete thrombosis of the external iliac and common femoral veins. The risk in using a percutaneous approach in this patient was the proximity to gluteal vessels (posteriorly), to bowel loops (anteriorly), and the possible embolization of neoplastic or thrombotic material during the tissue sampling. On this basis we decided to deploy a filter in the inferior cava vein before proceeding to an #endovascular #forceps #biopsy via a trans-jugular vascular access, in order to prevent pulmonary embolization.
