To understand what is meant by breathy voice, we need to first understand what is the function of the vocal cords. The muscles of the vocal cords, there are abductors and adductors. Adductors close the vocal cords and abductor muscles open up the vocal cords. Whenever there is a paralysis of the adductor muscles, the abductor muscles which open the vocal cords, they take over and they pull the vocal cords laterally resulting in a gap called as the glottis chink. Whenever there is large opening in the glottis that leads to a breathy voice. So we need to understand why this breathy voice has happened, whether it is due to an infection or due to an inflammation or due to allergy or due to trauma to the nerve, Based on that you have to treat that cause If it is due to an inflammatory condition, we have to treat that. If it is due to a neoplasm we have to address and find out which neoplasm is causing vocal cord paralysis and address it. If no cause can be founded, then also we have to investigate and found out if there is anything pressing upon the nerves, supplying the vocal cord, it is called as the recurrent laryngeal nerve. If the vocal cords is being compressed by a tumour either in the neck or in the mediastinum paralysis and once the abductor muscles take over, that results in a breathy voice. Wait for spontaneous resolution following inflammatory and infective conditions within 3 to 6 months and the voice is still breathy along with aspiration, then we need to do a procedure called a medialisation thyroplasty. We have to inject substances like fat or sclerosing agents or TEFLON into the weak vocal cord and increase the bulk and resulting in the medialisation of the vocal cords.
