Dr Justin E Davies and Dr Iqbal Malik are joined by Dr David Kandzari from Piedmont Hospital Atlanta, USA, for a live case demonstration at Hammersmith Hospital London, UK. In this demonstration they demonstrate the use of physiology to aid clinical decision making and appropriateness of PCI in the assessment of multi vessel coronary artery disease. Instantaneous wave-free ratio (iFR) is used to assess physiological severity, and iFR-Pullback or SCOUT is used to differentiate between focal and diffuse disease within the vessel. Finally, IVUS co-registration is performed using SyncVision. This technique fuses IVUS and angiographic images to simplify measurement interpretation and analysis. This case demonstrates the difference between angiographic and physiological severity, and clearly demonstrates that "angiographic tight" lesions are frequently not flow limiting. The Syntax 2 study (NCT02015832) is testing the differences in clinical outcomes from PCI in multi-vessel disease, when only treating lesions with physiologically significant lesion with modern drug eluting Synergy stents and IVUS optimisation, in comparison to a matched historical surgical cohort.
