Active Surveillance vs. Watchful Waiting

PCRI’s Executive Director, Mark Scholz, MD, explains the differences between two commonly confused concepts in prostate cancer nomenclature: “Watchful waiting” and “active surveillance.” 17: “Watchful waiting” is an older strategy that essentially means doing nothing unless the symptoms of metastatic disease begin to develop in form the bone pains in which case a man would undergo androgen deprivation (Also known as testosterone inactivating pharmaceuticals or TIP). This strategy is reasonable for elderly men who have fewer years at risk (since prostate cancer is so slow growing and metastatic disease is less likely even than dying from another cause) and for whom intense treatments are not an option. However, it would be inappropriate for anyone else since it bypasses the window of time in which there is an optimal chance of curing the disease. 1:21 “Active surveillance,” on the other hand, is based on the idea that you can watch localized prostate cancer on a strategic schedule, and if there is a change or if it progresses, then you can treat it before it spreads. This extends the option of forgoing treatment to younger men with low-risk forms of prostate cancer which opens the door to better treatments in the future if necessary, avoids the side effects of treatment for a longer period, and still maintains the ability to undergo treatment when there is an optimal chance of curing the disease.
