In order to get the true ending, you must complete both Episode P3 and Episode P4 with 100%. This involves going back to some of the chapters and selecting all of the dialogue choices. A chapter will display a red "Clear!" if you completed it 100%, otherwise it will show a yellow "Clear" meaning you will need to go back. The true ending shows somes developments of the character Sho Minazuki that are not seen in the regular story. It's a nice touch.
