Percutaneous Endoscopic Gastrostomy (PEG Tube) Insertion

The surgery begins with the insertion of an endoscope through the mouth down through the esophagus and into the stomach. The stomach is then insufflated with air and the surgeon's finger is pressed against the abdomen wall where the light from the endoscope is located. This indentation of the surgeon's finger is visible in the view through the scope. The Safe Tract Method is utilized to insert a 25 gauge needle, and as the needle is inserted, the surgeon pulls back on the needle to make sure no blood or air returns before the needle reaches the stomach. Once the needle pierces the stomach air bubbles are present in the syringe. A larger 16 gauge needle is inserted along side the 25 gauge needle and the 25 gauge needle is removed. The guide wire is inserted through the 16 gauge needle and grasped by the scope. The scope is withdrawn through the esophagus and the guidewire is pulled back, pulling the gastronomy tube down through the esophagus and through the abdominal wall. The scope is reinserted revealing the endoscopic view of the internal bumper in the correct position.
