While they are not usually harmful, they can be cosmetically distressing and sometimes indicate underlying lipid disorders. In this video, Dr. Ankit Gupta explains what xanthelasma is, why it occurs, and how it can be effectively treated. ✅ What is Xanthelasma? Xanthelasma are fatty deposits composed of cholesterol that build up under the skin, particularly in the thin and delicate skin around the eyelids. They may appear in people with high cholesterol, high triglycerides, or other lipid abnormalities, but they can also develop in individuals with normal blood lipid levels. These deposits may increase in size over time and affect both appearance and confidence. ✅ Why Does Xanthelasma Occur? The most common reasons include: High cholesterol levels (LDL – “bad” cholesterol). High triglycerides. Genetic predisposition. Lifestyle factors like poor diet, lack of exercise, or smoking. Systemic conditions like diabetes or metabolic syndrome. It is strongly recommended that anyone who develops xanthelasma should undergo a lipid profile test to check their cholesterol and triglyceride levels. Managing these levels through medication, diet, and lifestyle changes is crucial to prevent recurrence. ✅ Treatment Options for Xanthelasma There are several ways to treat xanthelasma, but not all are equally effective: Laser Treatment (CO2 or Er:YAG): Burns away the cholesterol deposit superficially. Can leave behind scars, pigmentation, or uneven skin texture. May require multiple sessions. Chemical Peels: Use acids to remove superficial layers of skin. Works only on the surface level. A small incision is made along the natural crease of the eyelid. The cholesterol deposit is carefully excised from the skin, dermis, subcutaneous tissue, and orbicularis muscle. Fine sutures are placed, which heal well with time. Within 1–2 weeks, most swelling resolves, and results become visible. ✅ Post-Treatment Care After surgical removal of xanthelasma: Mild swelling and redness may last for a few days. Antibiotic ointment and cold compresses help healing. Stitches (if non-dissolvable) are removed in 5–7 days. Patients should monitor their cholesterol and triglyceride levels regularly. Follow a healthy lifestyle – balanced diet, exercise, and prescribed medication to prevent recurrence. ✅ Lifestyle & Medical Management Even after removal, xanthelasma can recur if the underlying lipid levels are not controlled. To prevent recurrence: Get a lipid profile test done (cholesterol, triglycerides, LDL, HDL). Follow a low-fat, high-fiber diet. Include regular exercise in your routine. Take statins or lipid-lowering medications if prescribed by your physician.
