This is the second in a series of videos - 'deconstructing cataract surgery' - designed to help surgeons in training develop a solid foundation in cataract surgery technique. Learning to use an entry site as a valve is a fundamental skill in anterior segment surgery. Floating in the external ostium of the entry site during injection allows the internal ostium to act as a flap valve, whereas advancing the cannula into the paracentesis splints the internal ostium open. For the best control, use your dominant hand to control the cannula in the entry site by holding the distal end of the syringe like a pen, and injecting with your non-dominant hand. Kissing the external ostium of the main entry site with the irrigation port of your I/A is useful for a quick fill - particularly in vitrectomised eyes. Your postoperative check is on the table: make sure the eye is holding a good physiological pressure by testing with your index finger; use 2% fluorescein to check for entry site leaks if necessary; and, if ever you think about using a suture, you should probably use one.
