Today we are reviewing the anatomy of the SMAS (superficial muscular aponeurotic system). Watch this video to learn the important anatomy and the implications for facelift surgery! Mid-face Review Knowledge of the antomy of the mid-face region is vital to performing facial rejuvenation surgeries, non-invasive injectables, and head/neck reconstruction. The zygomaticus major muscle and zygomatic cutaneous ligament are essential structures encountered when performing a deep-plane facelift. A vital part of the deep-plane facelift is release of the zygomatic cutaneous ligament (e.g. McGregor's patch). Proper release of this ligament allows for maximal lift and natural results after facelift surgery. You can see the ligament running from the lateral nasal bone to the zygoma in this attached figure. Moreover, knowledge of the nerves and vessels in the area are paramount to avoiding complications. Temporal Anatomy: The layers of the temporal area often confuse residents, but it is vital to understand the anatomy of this area for facial rejuvenation surgeries, non-invasive injectables, and head/neck reconstruction. The first thing that confuses everyone is the nomenclature. To clarify: -Temporoparietal fascia (TPF) = Superficial temporal fascia -Temporali fascia = deep temporal fascia. --The deep temporal fascia itself has a superficial and deep layer. Once you grasp the nomenclature, the next important concept is location of facial nerve: The facial nerve is always deep to the SMAS. We know the SMAS is continuous with the platysma, TPF, and galea. As such, the frontal branch of the facial nerve is just DEEP to the TPF.
