This case describes a full-mouth rehabilitation combined with implant therapy in a geriatric patient, focusing on the successful restoration of function and aesthetics through the increase of OVD. 2. Case Description An 84-year-old female patient visited our clinic hoping to restore generalized teeth functional and aesthetic. Clinical and radiographic examinations revealed severe attrition and multiple tooth loss. A comprehensive treatment plan was formulated, involving a 5-6 mm increase in OVD and implant placement. Given the patients advanced age and the significant magnitude of OVD increase, a three-step staged approach was implemented. By integrating traditional techniques with digital workflows, an optimal treatment plan was established, and aesthetic final prostheses were successfully delivered following the planned OVD elevation. 3. Discussion The primary challenge in increasing OVD is ensuring the adaptation of the mandible and associated musculature. In this case, a meticulous analysis of the mandibular plane was conducted during the diagnostic wax-up and provisional stages. Proper orientation of the occlusal plane was established, and a minimum of three months was dedicated to the provisional phase to facilitate neuromuscular adaptation. This gradual transition was crucial for stabilizing the new maxillomandibular relationship in a geriatric patient. 4. Conclusion Through thorough preoperative planning and a systematic staged approach, full-mouth rehabilitation with increased OVD provided highly satisfactory functional and aesthetic outcomes. This case demonstrates that even in elderly patients, significant OVD restoration can be predictably achieved with careful monitoring and a combination of conventional and digital prosthetic protocols.
