In this video, we will be discussing a transcatheter strategy to close congenital portosystemic shunt (CPSS) associated with hypoplastic portal vein (HPV). CPSS is a rare congenital anomaly that results in abnormal blood flow between the portal and systemic circulation, which can lead to a number of complications including liver dysfunction, portal hypertension, and variceal bleeding. We will begin by reviewing the anatomy and pathophysiology of CPSS and HPV, as well as the current surgical and transcatheter options for the management of these anomalies. We will then discuss our experience with a transcatheter strategy, which involves the use of a combination of embolic agents, such as coils and plugs, to occlude the shunt and reduce blood flow through the abnormal vessels. We will present the case of a patient with CPSS and HPV who underwent this transcatheter strategy and discuss the technical aspects of the procedure, as well as the patient's clinical outcomes. We will also review the potential risks and complications of this approach, as well as future directions for research in this field. This video will be of interest to healthcare professionals involved in the management of congenital heart disease, particularly pediatric interventional radiologists, pediatric interventional cardiologists, and pediatric hepatologists.
