Ptosis is a drooping of the eyelid, and repair remains a challenge for surgeons despite more than a century of surgical refinements. This procedure video illustrates an algorithm-based approach to levator aponeurectomy during external ptosis surgery that accounts for the fact that the levator aponeurosis is extensile and small stresses cause it to lengthen. The surgeons apply a consistent amount of force to the aponeurosis during the procedure using a medical-grade spring scale that allows for standardization of the amount of stress placed between patients.
