Many times, patients are unaware of the specific surgeries advised to them by their surgeon. Understanding these procedures helps patients gain a better understanding of their treatment. Today, we will discuss what a right hemicolectomy is, when it is performed, and how it is done. Understanding the Gastrointestinal System Let’s start with an overview of the gastrointestinal (GI) system. The food pipe (oesophagus) leads to the stomach. After the stomach comes the pancreas and duodenum. The small intestine begins after the duodenum, leading to the large intestine, which starts from the cecum and appendix. The large intestine has two main sides: the right side and the left side. The right side of the colon includes the cecum, ascending colon, hepatic flexure, and part of the transverse colon. When is a Right Hemicolectomy Performed? A right hemicolectomy is performed when there is cancer in the right side of the colon, which includes the cecum, ascending colon, hepatic flexure, and right part of the transverse colon. This surgery involves removing the right half of the colon, and patients must be familiar with this terminology, as it may appear in endoscopy or CT scan reports. Surgical Procedure In addition to removing the cancerous part of the large intestine, the last 10 to 15 centimeters of the small intestine, which connects to the large intestine, is also removed. During cancer surgery, we focus not only on the tumor itself but also on ensuring clear margins (removing healthy tissue around the tumor) and checking the lymph nodes for cancer spread. The blood supply to the tumor is also considered, and the lymph nodes surrounding the blood vessels are removed to ensure complete removal of the cancerous tissues. Reconnection After Surgery After the right side of the colon and part of the small intestine are removed, the remaining part of the small intestine is reconnected to the transverse colon. This completes the procedure. Types of Surgery This surgery can be performed in three ways: Open Surgery: A large incision is made. Laparoscopic Surgery: Also known as keyhole surgery, it involves small incisions and the use of a camera. Robotic Surgery: A more advanced, precise method that involves robotic assistance. The core surgery remains the same, but the method of performing it varies. It’s like eating the same meal but using a fork, chopsticks, or your hands. How Do We Know Where the Tumor Is? The location of the tumor is determined through CT scans or PET scans, which show the exact position of the tumor. Endoscopy and biopsy also help identify the type of cancer and its location. These diagnostic tools guide us in deciding whether a right hemicolectomy is necessary.
