Increase intensity in small gradual calculated increments. Serious pathology must be ruled out first in all chest and rib cage pain. Thoracic Spine Disc Herniation Thoracic spine has a lower rate of disc herniation than cervical and lumbar spine. Symptomatic thoracic disc herniation is clinically rare. This rarity is attributed to the orientation, structure and function of the thoracic spine. Many thoracic disc herniations are asymptomatic and discovered incidentally with an MRI Anatomy: Intervertebral discs are located between the vertebrae. The intervertebral disc is composed of two materials: the outer hard fibrous ring called the annulus fibrosis, and an inner soft gelatinous core named the nucleus pulposus. The intervertebral disc functions to both absorb shock and allow flexibility of the vertebral column. As the body ages, the integrity of the intervertebral disc declines and causes the inner core of the disc to protrude through the outer layer. The effects of these will be either compression of the nerve roots or the spinal cord, giving rise to radicular symptoms. Pathophysiology of herniated discs is believed to be a combination of mechanical compression of a nerve by the bulging nucleus pulposus and local inflammation. Origin may be trauma or repetitive overload. Clinical presentation varies widely depending on location and characteristics. More common in upper thoracic spine at T1 or in the lower thoracic spine. Most common initial symptom is thoracic pain. Pain can be midline, unilateral or bilateral. Sharp shooting shock-like pain. Numbness, tingling and/or burning. Possible weakness. Nonspecific symptoms may exist including chest wall pain, epigastric pain, upper extremity pain, groin pain and lower extremity pain. Imaging: X-rays display the bone and joint while MRI displays the soft tissue. MRI is very sensitive and specific for diagnosing thoracic spine disc herniation. Majority of cases of thoracic disc herniation are successfully treated with conservative management. Though relatively rare, thoracic disc herniation should be considered in cases of chest wall pain and midback pain. Prevention: Thoracic spine strengthening, thoracic spine stretching, thoracic spine mobilization, pull-up bar hanging traction (Dead Hangs), posture correction exercises, stretching of chest, latissimus dorsi and serratus anterior muscles and strengthening of scapular retractor muscles (Rhomboids, middle traps and lower traps), posterior rotator cuff muscles (Supraspinatus, Infraspinatus and Teres Minor) and posterior head of deltoid. Chiropractic Care: The primary objectives of chiropractic treatment are the optimization of nerve flow and the re-establishment of correct skeletal biomechanics. Thoracic Spine T1-T12 Each thoracic vertebra articulates with ribs bilaterally. Twelve pairs of thoracic nerve roots. Twelve Intervertebral Discs (IVD). Located below the corresponding numbered vertebra. Intervertebral Disc increases spinal motion and provides the majority of the spine’s weight-bearing capacity. Function: Provides a measure of shock absorbing protection to the spinal column and appropriate stability for the spine during load bearing activities. The inferior surface of the superior vertebral body articulates with the superior surface of the inferior vertebral body through intervertebral (IV) discs.
