Many people get confused with seeds in diablo 2. For singleplayer, every time you press Play the game generates a new seed. Then all actions "advance" the seed and the items, shrines, monster spawns, and chests (Except Racks. These are tied to map) get created based on whatever that seed is. This is why when you use the "-seed" argument in your launcher and do the same actions, you can produce the same results (same shrine appearing, same rune dropping from a chest, etc). However, without this argument (which I don't use) the seeding always changes. The confusing part for people is why don't maps change then because maps are generated based on seeds. And that is because once a map is generated it gets put into the d2s (character) file. So it is no longer being regenerated every game even though the seed is changing. Only upon difficulty change does it shift in singleplayer.
