It is a very common symptom that women experience. However, it is a very subjective symptom. It is difficult to objectively measure the amount of blood lost during the period but anywhere between 30-60 ml is considered normal and if it exceeds 80 ml then that is considered as a heavy period. A detailed history is taken to understand heaviness. How many pads you use? Whether you combine it with tampons? Is it the extra large that you use? Do you get up in middle of the night to change your pads? Do you experience flooding that is soaking through your clothes? How often you do you need to change the bed linen or clothes etc. Association of clots with flooding is also an indication of heavy period. Fatigue, shortness of breath and tiredness also indicate heavy blood loss. Pain is a common symptom but if the pain is getting worse month on month or if it is starting prior to the period itself it may need to be checked out. Many times it is difficult to pinpoint the cause for heavy period, some conditions like Poly Cystic Ovarian Syndrome, fibroids, endometrial polyps which are small grape like growths within the endometrium, coagulation problems , and hypothyroidism may contribute to heavy menstrual bleeding, very rarely it may indicate endometrial cancer or cancer of womb. Detailed history is taken and baseline blood tests such as thyroid and hemoglobin. Scan is avoided something abnormal is seen during the examination. Mostly treatment is not necessary. Watch and observe is done. If treatment is required one starts with simple medications to reduce the flow, this helps up to 50-70%. If simple medications don’t work Medication with hormones are tried. If this does not help then intra uterine system is tried. The last resort is endometrial ablation or hysterectomy. Heavy periods associated with pain and getting worse month on month may need to be checked out.
