In this video, I discuss ptosis, a drooping of the upper eyelid. This is important for opthalmology trainees, medical students, those sitting USMLE, medical finals and part 1 FRCOphth. Ptosisis a common problem. But often patient's may not present. One of the causes of ptosis is Horner's syndrome.This is due to interruption of the sympathetic nervous system. • If horner’s syndrome is congenital. There will be heterochromia with the affected eye lighter than the non affected eye. This is because iris pigmentation is under sympathetic control prior to two years.There is also lack of sweating on the affected side of the face. A third important cause of ptosis is a third nerve palsy. It is a very important not to miss this sign as third nerve palsy can be the first sign of a posterior communicating aneurysm. The oculomotor nerve supplies innervation to the levator palpebrae superioris so damage to this nerve will of course give ptosis. Another cause of ptosis is myasthenia gravis. This is a myogenic cause. This will cause variable and fatigue able ptosis. This is an autoimmune condition caused by autoantibodies against the acetylcholine receptor.
