Cataract surgery in a case of severe phacodonesis with scleral fixation using scleral pockets

Phacodonesis is a surgical challenge with high risk for early intraoperative complications such as severe zonulolysis, posterior capsule tear, vitreous loss and drop of lens particles to the vitreous, or late complications of IOL subluxation. When the ophthalmic surgeon deals with mild to moderate phacodonesis he must support the capsular bag with devices such as hooks or CTR. When there is a severe phacodonesis, a fixation of the IOL is needed. In this video, there was not enough capsular support to secure the IOL so at the end of the surgery the IOL was moving from side to side with high risk for early subluxation. The surgeon created 2 Hoffman's scleral pockets and fixated the IOL with 2 prolene suture (9-0). The surgery was done by Dr. Ori Mahler, Israel.
