A 7 mm cystic lesion is identified in the right lobe of the thyroid, featuring a hypoechoic mural nodule with marked internal vascularity on color Doppler imaging. While there are no calcifications, the hypoechoic nature and vascularity of the mural nodule raise suspicion for malignancy, particularly papillary thyroid carcinoma (PTC) or its follicular variant. Benign possibilities include a hyperplastic or colloid nodule, though these typically lack such vascularity and hypoechoic features. A thyroid cyst with granulation tissue or clot is another consideration if recent hemorrhage is suspected. Fine-needle aspiration (FNA) biopsy is crucial for definitive diagnosis. Based on its hypoechoic, vascular nature, the lesion likely falls into a high-suspicion TI-RADS category (TR4/TR5). If malignant, early-stage PTC has an excellent prognosis with timely treatment.
