Occlusive lesion of Common femoral artery at the sheath site! Antegrade Angioplasty, Hamo-technique! Post intervention on LAD while checking the femoral angiogram to see the sheath insertion, it was found that the sheath was occlusive. A severe CFA lesion , (chronic lesion with developed collaterals ) IA ntg to relieve any sheath induced spasm. The stenosis nor the Flow didn’t improve . 1. Keep the wire in , 2. Advance a 7.0x40 mm DCB balloon in the sheath at the site of the stenosis, 3. then pull the sheath out the artery with gentle finger pressure above the vessel with the balloon inside, 4 inflate the balloon at nominal pressure for a minute 5. Deflate the balloon 6. Re Advance the sheath back over the deflated balloon ( use the balloon as a dilator) 7. Pull balloon 8.give Ntg 9. Check pressure in sheath 10 final angiogram
