Speaker: Dr. Shivakumar Thiagarajan Topic: Management of infra-temporal fossa In this video, Dr. Shivakumar talks about: a. Anatomical Considerations in Infratemporal Fossa Surgery: Understanding the spatial relationships, particularly the location of the foramen ovale relative to the internal carotid artery, is crucial for safe surgical clearance. b. Surgical Classification and Resectability of ITF Tumours: Tumour classification systems, help determine resectability, with involvement of the pterygomaxillary fissure, infraorbital fissure, and intracranial extension often indicating unresectability due to the difficulty of achieving R0 resection and the risk of widespread disease. c. Intraoperative Techniques to Minimize Complications in ITF Surgery: Meticulous surgical technique, including early identification and ligation of the parotid duct to prevent fistula, and controlled use of suction to maintain a clear surgical field without excessive blood loss, are essential for minimizing the risk of complications during ITF clearance. d. Managing Internal Maxillary Artery and Pterygoid Venous Plexus Bleeding: Early identification and clipping of the internal maxillary artery posterior to the mandibular condyle can significantly reduce intraoperative bleeding. Managing pterygoid venous plexus bleeding, a major challenge, requires intermittent packing and precise cauterization with bipolar diathermy in a clear surgical field. e. Avoiding Facial Nerve and Internal Carotid Artery Injury During ITF Clearance: Protecting the zygomaticotemporal branch of the facial nerve during cheek flap elevation by staying close to the zygoma is crucial. Avoiding injury to the internal carotid artery necessitates strict adherence to the surgical plane at the level of the foramen ovale and avoiding dissection deeper to it. f. Reconstruction Considerations and Functional Outcomes After ITF Surgery: Extensive resections for ITF tumours often require complex reconstruction to address swallowing and cosmetic issues. Careful surgical technique and appropriate reconstructive methods are vital to minimize functional deficits and optimize the patient's postoperative quality of life.
