Maria is showing an eccentric exercise for the hip flexors to improve hip extension using the Thomas test as a reassessment tool. C/O findings: The patient experienced a forced extension trauma in September 2019 (playing soccer). Since then he feels pain in the left groin and a limitation of his hip extension because of subjective stiffness. His symptoms aggravate with playing soccer, running and exercises in the gym that emphasize hip extension esp. performing lunges. Symptoms also increase after stretching exercises into extension. During the physical examination the functional tests showed a restricted extension in the hip and an increased mobility in extension of the lumbar spine, especially during walking, lunges, and active lumbar extension in standing. There was no muscle weakness identified around the hip. The passive physiological movements were restricted in flexion, rotation and extension. Flexion and FABER test reproduced the typical symptoms. First treatment was a passive mobilization technique that increased the mobility in flexion and extension during reassessment, but the functional tests stayed the same. As one of the main goals of the patient was to gain more pain free hip extension, Maria introduced an eccentric hip extension exercise and used the Thomas test as a reassessment tool as described by Vigotsky et al 2016.
