Patient had trauma with 5 clock hour cyclodialysis cleft causing hypotony and maculopathy. Cleft was closed after 4 laser sessions with improvement of vision. Video is at double speed. Lens: Ritch Trabeculoplasty Laser Lens, Goldmann 3 mirror lens, or SLT lens Power: 500 mW Spot Size: 200 um Duration of Burn: 0.5 sec Burns: 150 Key points: The purpose of cyclodialysis cleft laser is to induce synechiae, to cause the peripheral iris to adhere to the angle. Therefore, need many laser shots to the peripheral iris and angle to induce adherence. This procedure may need to repeated multiple times, and there may be pain during the procedure. After the first session, start prednisolone 4 times a day. Follow up in 1 week, and explain that if there is pain or discomfort, to return prior to 1 week (which indicates closure of the cleft and increased IOP). If at 1 week visit there is no effect, then can repeat laser at that time. Christopher Teng, MD
