Tibialis anterior tendonitis or tendinopathy is a less common but impactful condition that causes pain and tenderness along the front of the ankle or shin, typically near the tendon’s insertion on the medial cuneiform and first metatarsal. This condition often develops from repetitive or excessive dorsiflexion activities—such as uphill walking, running, or stair climbing—and may present with anterior ankle stiffness, weakness, or aching that worsens with activity. Over time, the tendon may undergo degenerative changes, leading to chronic discomfort and impaired function. In physical therapy, treatment focuses on reducing tendon irritation, restoring load tolerance, and improving lower limb mechanics. In this video, I’ll demonstrate a soft tissue mobilization technique to reduce tension and improve mobility in the tibialis anterior muscle belly and tendon. I’ll also guide you through an isometric heel walk, a useful low-load activation exercise, and a resisted dorsiflexion exercise that strengthens the muscle through its full range of motion. These interventions aim to calm symptoms, progressively reload the tendon, and support long-term recovery.
