This video demonstrates a routine phacoemulsification case complicated by a tear in the posterior capsule. The incident occurred while trying to irrigate adherent cortical fibers to the posterior capsule. The tear was large and management started by immediate injection of cohesive and dispersive OVD. Since no vitreous was present in the AC, I decide to implant the single-piece IOL inside the capsular bag and do reversed-optic capture. This was followed by Irrigation / Aspiration and bringing the pupil down by Acetyl Choline injection inside the anterior chamber. On the first postoperative day, the case looks uneventful and the patient enjoyed good visual outcomes.
