From our Complete Cataract Case series, this is a video of a patient with pseudo-exfoliation syndrome which can make the surgery more challenging. How can we predict the degree of zonular laxity as we start the case? The keys are the pupil dilation, the variability in the anterior chamber depth when comparing pre-op seated position at the slip lamp versus supine on the operating room table, and the amount of anterior capsule wrinkling when performing the capsulorhexis. Pseudo-exfoliation syndrome is common enough that I encounter at least a few patients per week with this condition and some weeks it is many patients.
