DRCR Protocol T - Avastin vs Lucentis vs Eylea for Diabetic Macular Edema

Protocol T of the Diabetic Retinopathy Clinical Research (DRCR) was a randomized controlled trial of 660 patients that compared the effectiveness of Avastin (bevacizumab), Lucentis (ranibizumab), and Eylea (aflibercept) for the treatment of diabetic macular edema (DME). For patients with a starting visual acuity of 20/40 OR BETTER, all 3 drugs showed the same effectiveness. For patient starting 20/50 OR WORSE, Eylea demonstrated a greater improvement in visual acuity compared to Avastin (p=0.0001) and Lucentis (p=0.03) at 1 year. At 2 years, Eylea remained statistically better than Avastin only. While Eylea seems to be the superior option for DME in patients with vision 20/50 or worse, one of the major limitations is its cost, making it infeasible for some populations. Protocol AC is an ongoing trial assessing if starting Eylea after initial Avastin will affect long-term visual outcomes or if it is necessary to start with Eylea to obtain to best vision.
