This is a Pilomatricoma, a benign, hair follicle tumor. Pilo: resembling or composed of hair Matricoma: Benign tumor of the hair matrix They can occur at any age but tend to be seen more commonly in younger people. The most common location is the head and neck but can occur anywhere. They can be skin color to red and due to calcifications, the lesions can be quite hard. Diagnosis is usually easy by a skilled dermatologist, however, rarely malignant change is possible and therefore the lesion is usually biopsied. Microscopically: 1. Hallmark feature: Shadow or ghost cells. These are the keratinized remnants of the basaloid cells that are present at the periphery of the tumor. They appear like the ghosting or shadow of cells that once were (hence the name). 2. Basaloid cells appear quite primitive, can often show atypia, with mitotic figures. They are also positive for p16 immunostain. This is a diagnostic pitfall for head and neck FNA, which may be incorrectly diagnosed as HPV related basaloid SCC. 3.
