He emphasized the importance of a quick 30-second history-taking to determine whether the patient is diabetic, for how many years, and the medications they take, whether pills or insulin, and whether they have taken them that day. He then highlighted the need to check for a major complication of hyperglycemia: Diabetic Ketoacidosis (DKA). Immediate intervention involves administering Ringer’s lactate and rechecking blood sugar in 10 minutes, followed by assessing symptoms of hyperglycemia, such as excessive thirst, frequent urination, fatigue, nausea, vomiting, and confusion. The next step is deciding the appropriate insulin management, short-acting, long-acting, or premixed insulin, administered via intramuscular, intravenous, or subcutaneous routes. He further explained emergency management strategies for severe hyperglycemia. One option is to administer 4 units of regular insulin three times a day (TDS) every 4-6 hours, with frequent monitoring to ensure blood sugar does not drop below 200 mg/dL, at which point a doctor should be consulted. A more effective approach involves preparing an infusion by mixing 50 units of regular insulin in 45 mL saline and administering it through a syringe infusion pump at an initial rate of 2-3 mL per hour. Dr. Shukla also reminded students of the normal blood sugar ranges, fasting levels should be between 70-100 mg/dL, while random blood sugar should ideally be around 150 mg/dL. Dr. Shukla concluded by discussing the Sliding Scale Method for insulin administration, where insulin dosage is adjusted according to blood sugar levels. If the sugar is up to 150 mg/dL, no insulin is required; if between 150-200 mg/dL, 2-3 units are given; if between 200-300 mg/dL, 5 units are administered, and the dosage continues to increase proportionally. However, he stressed that insulin management must always be individualized, as what works for one patient may not be suitable for another.
