Endoscopic Spine Surgery for a Far Lateral disc herniation
In this video, I discuss my personal technique for a far lateral discectomy where the herniation is located outside of the foramen, no foraminoplasty is necessary, and the essential maneuver required to visualize the herniation is to roll the endoscope inverted and look down on the herniation. When done correctly, this technique facilitates the retrieval of far lateral and extra foraminal disc herniations with essentially zero blood loss or trauma to surrounding soft tissues. The key here is to appreciate how to go inverted and not get disoriented!
