What are Fibroids? Fibroids are non-cancerous growths that develop in or around the womb (uterus). The growths are made up of muscle and fibrous tissue and vary in size. They're sometimes known as uterine myomas or leiomyomas. Here are some key tips for how to choose the best fibroid management approach for you. Firstly, addressing symptoms is important but then, knowing goals about how you would use your womb in the future is majorly important for deciding on how to manage: 1. 𝐍𝐨 𝐏𝐥𝐚𝐧𝐬 𝐭𝐨 𝐜𝐚𝐫𝐫𝐲 𝐚 𝐛𝐚𝐛𝐲 𝐢𝐧 𝐭𝐡𝐞 𝐟𝐮𝐭𝐮𝐫𝐞 Options like hysterectomy, uterine fibroid embolization (UFE) and/or medical management are recommended. Note that medical management is especially pursued if a patient is not a good candidate for surgery due to say having heart, kidney or liver failure etc. UFE isn’t recommended for persons desiring to carry a baby in the future! UFE is performed by a specialized radiologist known as an interventional radiologist. 2. 𝐃𝐞𝐬𝐢𝐫𝐞𝐬 𝐭𝐨 𝐜𝐚𝐫𝐫𝐲 𝐚 𝐩𝐫𝐞𝐠𝐧𝐚𝐧𝐜𝐲 𝐢𝐧 𝐭𝐡𝐞 𝐟𝐮𝐭𝐮𝐫𝐞 𝐨𝐫 𝐚𝐭 𝐥𝐞𝐚𝐬𝐭 𝐰𝐚𝐧𝐭 𝐭𝐨 𝐡𝐚𝐯𝐞 𝐭𝐡𝐞 𝐨𝐩𝐭𝐢𝐨𝐧 𝐭𝐨 𝐝𝐨 𝐬𝐨? Myomectomy (fibroid removal surgery) is the recommended gold standard to treat symptomatic fibroids in someone who desires to preserve the integrity & pregnancy carrying function of the uterus. Whether to do an Open myomectomy, laparoscopic myomectomy, or Hysteroscopic myomectomy will depend on the total number of fibroids, their size & the location of the fibroid(s). Sometimes, patients have a Hysteroscopic myomectomy plus one of the other approaches at the same time. Radiofrequency ablation (RFA) of fibroids is also an option but that is really dependent on the total number, size & location of the fibroids. Determining if this option is right for you requires consultation with a surgeon who is trained to perform this procedure. Never be afraid to have a frank discussion about those goals above. Sometimes, in order to have the best chance at preserving the pregnancy carrying capacity of the womb, your doctor may first have you take medicines like depot-Lupron or oriahnn for a few months, to shrink the fibroids to better allow the Doctor to access your fibroids and decrease your risk of ending up with an emergency hysterectomy. Seeking 2nd opinions about surgical management is also recommended if you are not comfortable with the options recommended to you.
