Dr Maran explains what a hiatus hernia is in this video is. He explains that hiatus literally means a hole. The chest region is separated from the abdomen by the diaphragm. There is a hole or a hiatus in the diaphragm through which food pipe and the greater vessels like aorta traverse. A small part of the esophagus enters through this hole and connects to the stomach. This diaphragm has a sling-like structure that protects it from sliding up into the chest region. Dr Maran says that sometimes this sling-like structure becomes loose and the stomach can pass into the hole and move up through the hiatus hole into the chest region. He says that this condition is called a hiatus hernia. Dr Maran outlines that there are two types of a hiatus hernia. If the food pipe and part of the stomach all move through this hiatus and slide up and down, the type is called a Sliding hiatus hernia. If on the other hand, only the stomach moves through the hiatus, the type is called a fixed or a paraesophageal hiatus hernia. Dr Maran says that a fixed hiatus hernia causes more complications than a sliding hiatus hernia. Dr Maran then proceeds to enumerate the symptoms caused by the presence of a hiatus hernia. The most symptoms are Heartburn or GERD. Most can be without GERD also. He also says that not all people with a hiatus hernia develop symptoms. Sometimes ulcers can be formed in the stomach layer that has moved up. These ulcers can cause bleeding and eventually can cause anaemia if left untreated. Dr Maran says that fixed hiatus hernia can cause complications like the rotation of stomach called Volvulus. He says that many factors like obesity, pregnancy, chronic cough, wheezing, constipation, etc, can cause a hiatus hernia. Anything that can cause intra-abdominal pressure can cause a hiatus hernia. Dr Maran explains that if a hiatus hernia is not giving any symptoms or trouble, there is no need to worry. If symptoms are present, he recommends treatment. The gold standard diagnostic method of a hiatus hernia is Endoscopy. He recommends changing the lifestyle by losing weight, reducing fat, keeping fit, reducing intra-abdominal pressure in general, treating chronic cough etc. Even after a lifestyle change, if a hiatus hernia shows symptoms, then surgery can bring relief. Laparoscopic hernia surgery can be performed where the sling of the diaphragm is tightened. If complications like stomach rotation or volvulus are also reported, they can be corrected through surgery.
