Around the Table Dissection in Proliferative Diabetic Retinopathy with Tractional Retinal Detachment
Around the table membrane dissection in patients with PDR & TRD with extensive fibrovascular proliferation consists of two steps. 1. Separate the membrane at the optic nerve head to create a plane of dissection. 2. Create a wedge between central and peripheral retina by extending the dissection to the outside and induce the PVD in the peripheral retina. In this way the membrane can be attacked from both the inside and the outside. In most cases, the dissection can be completed using the vitrectomy cutter alone. In this video, we demonstrate the use of this technique in a 48 years female with PDR, TRD and extensive fibrovascular proliferation.
