In 1987, a radioactive source containing cesium-137 was abandoned in a demolished building in Goiânia, Brazil. Over the next 16 days, it passed from hand to hand — from a scrapyard to multiple homes — contaminating an entire city and exposing more than 112,000 people. This is the Goiânia radiation accident, one of the worst nuclear disasters in history — and one of the most preventable. Five institutions received written warnings about the abandoned source. One of those warnings used the word “bomb.” Every warning was filed. None were answered.
