Fees paying to obtain this service No charge Period taken to provide the Service (Ordinary Service and Priority Service) 03 weeks, if all documents are completed Necessary Supporting documents Application certified by the last employer, “B” cards relevant to membership numbers, National Identity card and a copy of the National Identity card certified by employer, a copy of the bank pass book; and Marriage certificate/ original of register of marriage or the original marriage certificate certified by Additional Registrar of marriages In case of a foreign marriage certificate, it should have been certified by the embassy of Sri Lanka in that particular country Staff Officers in charge of the Service Designation 00:00 00:25 00:35 Division Telephone Fax E-mail Deputy Commissioner of Labour EPF Payment Division 011-2369214 011-2368904 011-2368911 Exceptions, instances not included above and special information If there are shortcomings in the documents, such matters can be discussed with the Deputy Commissioner of Labour or Assistant Commissioner of Labour of the Labour office to which the application is handed over. සුදුසුකම් සේවයෙන්ඉවත් වූ දින සිට මාස 03 ඇතුළත විවාහය ලියාපදිංචි කර තිබිය යුතුයි.නැතහොත්, විවාහවූ දින සිට වසර 05 ඇතුළත සේවයෙන් ඉවත් වී තිබිය යුතුය. සැලකිය යුතුය :- ප්‍රතිලාභ ලබා ගන්නා අවස්ථාව වන විට සේවයෙන් ඉවත් වී තිබීම අනිවාර්යය වේ. ඉල්ලූම්පත් ඉදිරිපත් කිරීමේ ක්‍රියාවලිය (ඉල්ලූම්පත් ලබා ගත හැකි ස්ථාන, ඉදිරිපත් කළ යුතු ස්ථානය, කවුන්ටරය හා වේලාවන්) ඉල්ලූම්පත් ලබා ගත හැකි ස්ථාන :- ප්‍රධාන කාර්යාලයේ බිම් මහලේ ඇති කවුන්ටරයෙන් හෝ සියලු දිස්ත්‍රික් කම්කරු කාර්යාල හා උප කම්කරු කාර්යාල මඟින් ලබා ගත හැක. එමෙන්ම මෙතැනින් අවශ්‍ය ආකෘති පත්‍ර බාගත කර ගත හැක. ඉල්ලූම්පත් ලබා ගැනීමට ගෙවිය යුතු ගාස්තු. :- ගාස්තු අයකිරීමක් සිදු නොකරයි. ඉදිරිපත් කළ යුතු වේලාවන් :- සම්පූර්ණ කරන ලද අයදුම්පත සාමාජිකයා පදිංචි දිස්ත්‍රික්කයේ ලඟම පිහිටි දිස්ත්‍රික් හෝ උප කම්කරු කාර්යාලය වෙත සතියේ වැඩකරන දිනකදී කාර්යාල වේලාව ඇතුළත සාමාජිකයාම පැමිණ භාරදිය යුතු වේ. සේවාව ලබා ගැනීමට ගෙවිය යුතු ගාස්තු ගාස්තු අයකිරීමක් සිදු නොකරයි. සේවාව ලබා දීමට ගත වන කාලය (සාමාන්‍ය සේවා හා ප්‍රමුඛ සේවා) සියලු ලේඛණ සම්පූර්ණ කර ඇත්නම් සති 03ක් ඇතුළත අවශ්‍ය සනාථ කරන ලියකියවිලි අවසන් සේවායෝජක විසින් සහතික කරන ලද අයදුම්පත, සාමාජික අංක වලට අදාල B කාඩ්පත්, ජාතික හැඳුනුම්පත සහ සේවා යෝජක විසින් සහතික කරන ලද ජාතික හැඳුනුම්පතෙහි පිටපතක්, බැංකුගිණුමේ පිටපතක් සහ; විවාහ සහතිකය/ විවාහ ලේකම් පොතේ මුල් පිටපත හෝ අතිරේක විවාහ රෙජිස්ට්‍රාර් සහතික කරන ලද විවාහ සහතිකයක මුල් පිටපත විදේශ රටක විවාහ සහතිකයක් නම් එම රටේ ශ්‍රි ලංකා තානාපති කාර්යාලයෙන් සහතික කළ යුතුය. සේවා භාර මාණ්ඩලික නිලධාරීන් තනතුර අංශය දුරකථනය ෆැක්ස් ඉ-මේල් නියෝජ්‍ය කම්කරු කොමසාරිස් සේ.අ.අ. ගෙවීම් අංශය 011-2369214 011-2368904 011-2368911 ව්‍යතිරේක හෙවත් ඉහත අවශ්‍යතාවලින් බැහැර වන අවස්ථා හා විශේෂ තොරතුරු ලියවිලි වල යම් අඩුපාඩුවක් වේ නම් අයදුම්පත භාර දෙනු ලබන කම්කරු කාර්යාලයේ නියෝජ්‍ය කම්කරු කොමසාරිස් හෝ සහකාර කම්කරු කොමසාරිස් හමුවී අදාල ගැටලු සාකච්ඡා කළ හැක.
