Speaker: Dr. Radhika K Topic: Early Postoperative Challenges in Microvascular Free Flap Surgery In this video, Dr. Radhika K talks about: a. Early postoperative complications These can be non-microvascular, like acute neck bleeds and hematomas causing pedicle compression, or microvascular, presenting as venous or arterial insufficiency. Prompt recognition of these issues is crucial for timely intervention. b. Prevention strategies in free flap surgery Meticulous preoperative planning, including thorough patient assessment and flap selection, is key. Intraoperative measures like careful pedicle positioning, vessel quality assessment, and proper drain placement are also vital. c. Postoperative monitoring and early detection of free flap complications Close monitoring in the ICU for the initial days, utilizing prick tests and Doppler assessments, is essential for early detection. Vigilance for changes in flap color, turgor, and bleeding patterns is critical, especially in the first five days due to neck movements. e. Salvage management of failing free flaps For non-microvascular issues like bleeding, prompt re-exploration to identify and control the source is necessary. Microvascular complications may require local or full re-exploration, thrombectomy, and revision of the anastomosis, with heparinization considered in some cases. f. The importance of timing and backup plans in free flap salvage Early re-exploration (within 5 hours for microvascular issues) significantly improves flap salvage rates. Having a well-defined backup plan, such as the galic flip for venous congestion or alternative vessel options, is crucial when the primary flap fails.
