Hysteroscopy can be done in the office but if your doctor decides that you are not a candidate for an office procedure, your doctor may offer you a hysteroscopy with D&C in the operating room. 2. Common reason for the hysteroscopy with D&C may be • Stenotic cervical os (hard to dilate the cervix in the office) • Removal of submucosal fibroids • Numerous polyps • High risk for bleeding • Multiple medical co-morbidities • Patient unable to tolerate the office procure due to pain or severe anxiety • Remove remaining tissue after a miscarriage • Remove a dislodged IUD • Evaluate the cause of recurrent miscarriage • Remove uterine adhesions Common risks of the D&C procedure • Pain • Bleeding • Infection • Scar tissue formation (Asherman syndrome) • Uterine perforation o When a hole is accidently made in the uterus by a surgical instrument, this is rare. o You may need additional surgery to look in the abdomen to make sure you are not having an internal bleeding or the organs around your uterus were not injured o The uterus usually heals on its own • Reaction to anesthesia • Tears in the cervix Procedure A hysteroscopy is an instrument that has a camera and it is used to look in your uterus to see if you have any abnormalities • Polyps • Fibroids The hysteroscopy is done first. A speculum is placed in your vagina A tenaculum is placed on the cervix and then the hysteroscopy is gently inserted into your uterus while saline is instilled into the uterus Your cervix will be dilated to allow the hysteroscopy to slide inside the uterus. The saline or a hysteroscopy fluid medium will be infused into your uterus to distend the uterine cavity allowing your gynecologist to see the uterine fundus. Pictures are taken. If fibroids or polyps are seen they are removed The hysteroscopy is removed. A curettage or scraping of the uterine lining may done depending upon what the doctor sees in your uterine cavity. If you had a miscarriage the obstetrician may perform a vacuum assisted curettage The tissue is sent to the pathologist for evaluation. The tenaculum is removed The doctor will check for bleeding. The speculum is removed The procedure is finished. After the procedure 1. You doctor may tell you to avoid tampons, douching or intercourse for several days to prevent infection. 2. You have some vaginal bleeding and uterine cramping but these symptoms should resolve in 1-2 days. 3. Call you doctor if you have a fever or if your pain is not improving after 48 hours 4. If you are still having menstrual cycles, the stress of surgery may cause your cycle to start earlier than normal. Do not be alarmed. This is common.
