Enlarged Adenoids The adenoids are glands that sit behind the nose above the roof of the mouth They commonly enlarge in children – usually between three and seven years of age – sometimes till later. Adenoids become enlarged due to an infection or allergy. When they enlarge, they obstruct the breathing passage of the nose. This may lead to thick nasal discharge and nasal blockage. As a result the child may snore and have breathing problems leading to Sleep Disordered Breathing. In cases with persistent nasal blockage, mouth breathing, recurrent ear infections an Adenoid surgery may be advised. ADENOIDECTOMY Is done under General Anaesthsia Is done from inside the mouth, sometimes by observing through the nasal passage using and endoscope – there is no cut or incision on the face. Can be done using a Curette, a Coblator, or a Microdebrider Surgery lasts about 30 – 45 minutes Post operative recovery is usually uneventful apart from a soreness in the throat that lasts a few days Normal food can be eaten a day or 2 after the surgery What can happen if surgery is not performed in properly indicated cases? Prolonged enlargement of the Adenoids causes multiple problems – 1. Snoring 2. Disturbed sleep- twisting, turning, apnoea (stoppage of breathing), breathing difficulty, awakenings 3. Bed wetting at night 4. Daytime sleepiness, lethargy 5. Craniofacial abnormalities – poor muscle tone in facial muscles, elongated face, poor maxillary growth 6. Behavioural problems – Hyperactivity, somatization, depression, atypicality, aggression, and abnormal social behaviors 7. Cognitive Impairment a. Learning, memory b. Language, verbal fluency, and phonological skills c. Concept formation, analytic thinking, and verbal and nonverbal comprehension d.
