Case description: A 56-years-old patient came to our clinics with a history of blurred vision in the right eye. Two weeks ago, the patient underwent a complicated cataract surgery at another eye center. The intraocular lens was implanted in the sulcus. The ocular examination revealed a prolapsed vitreous body into the anterior chamber. We decided to perform a planned anterior vitrectomy to remove all the vitreous strands from the anterior chamber and reduce the risk of its complications. What is the main goal of anterior vitrectomy? The goal here is to remove the prolapsed vitreous from the anterior chamber as well as from the incisions in order to stabilize the already implanted lens and to prevent any further complications such as retinal detachment, etc. You can do this type of surgery using coaxial or bimanual technique, for me I prefer the bimanual technique since it gives a better control and more effective way to remove the vitreous. The vitrector should be set for a high cut rate between 500 and 800 cpm with low to moderate aspiration.
