The advantage of using the tearing technique for creating the initial primary rhexis is demonstrated in this video. In the classical shearing technique of rhexis creation the anterior capsular flap is folded and then the capsule is torn in a curvilinear manner, however in the tearing technique the capsule is held flat without folding and then it is pulled in a centripetal manner resulting in a circular tear. Although this technique is definitely less controlled , it is still valuable in creating the small primary rhexis in the swollen intumescent lenses. This video also demonstrates the advantage and disadvantage of a large rhexis during phacoemulsification. The larger rhexis significantly eases the process of nucleus division and emulsification but also results un controlled movement of free floating nuclear fragments which can cause mechanical damage to the corneal endothelium.
